The Physician Orders for Life-Sustaining Treatment (POLST) form is a tool for end-of-life planning. The form ensures that a patient's treatment wishes are known and will be followed by health care professionals during a medical crisis, when the patient cannot speak for themselves.

MOLST and POLST are two acronyms defining medical orders. The MOLST is the Medical Orders for Life-Sustaining Treatment and the POLST is the Physician Orders for Life-Sustaining Treatment. They're both the same thing, but in different states they call them by those two different names.

Governor Parson, on July 6, 2023, signed two measures SB 45 and HB 402, both granting recognition to TPOPP/POLST in Missouri, along with other state POLST forms, including Kansas, as long as those orders meet the provisions of the existing Missouri OHDNR statute.

The model created an Iowa version of the national POLST (Physician Orders for Life Sustaining Treatment) movement. The local IPOST projects were legislatively required to report to a statewide advisory council charged with making recommendations to the State Legislature.
