Get Ds-2019 Scholar Form - University Of South Florida

USF reviews incoming freshmen via their self-reported academic record (SSAR). You must submit your SSAR to be considered for admission.

In order to be issued an I-20 or DS-2019, you must show a valid passport and proof of liquid funds. All documentation is submitted via iStart. You will receive an email once your document is created. The document will be mailed directly to you as you must have the original I-20 or DS-2019 for your visa appointment.

USF's school code is SFR214F00608000. Your SEVIS record must be transferred to USF within your 60 days after completing all degree requirements or the last day of your OPT.

Official high school and college transcripts must be submitted to the University of South Florida before your first day of classes. Remember, your offer of admission is contingent upon our receipt and evaluation of your transcript. Make sure to submit official transcripts separately from all institutions attended.

USF requires freshman applicants to submit official results of at least one college entrance exam (SAT or ACT). USF does not currently require or consider the optional Essay section of the SAT or the ACT for the admission or scholarship review processes.
