Get Midyear Grades Wake Forest Form

An official transcript of your high school grades must accompany the Secondary School Report. The Secondary School Report must be completed and signed by your college counselor. School counselors are asked to submit a written evaluation along with the Secondary School Report.

Please submit this form as soon as mid-year senior grades are available. Complete the following regarding the applicant's academic performance for the first semester/trimester of the current year. Attach your own grade report form or copy of the high school transcript.

Wake Forest University admissions is most selective with an acceptance rate of 25%. Half the applicants admitted to Wake Forest have an SAT score between 1350 and 1500 or an ACT score of 30 and 33.

The mid-year report shares your student's performance at the midpoint of the school year, usually represented by first semester or trimester grades. Keep in mind that not every college requires the mid-year report. Refer to the requirements grid for information about which colleges do and do not use this form.

Some schools require the Mid Year report to be submitted via the application platform used to apply — Common App, Universal App, the institution's online app. Some schools, however, require a separate form to be signed and mailed. It all depends. Be sure you are well informed of the school's request.

All applicants for first-year admission who have not already graduated from secondary school should submit a mid-year report with official grades from the first marking period of the current academic year as soon as those grades are available.

The midyear report is a form that lists all your grades for the first half of 12th grade. Your high school guidance counselor fills out the report and submits it with your updated transcripts.
