The group should have a letter on organizational letterhead with: The names of the children on the trip and their primary address, phone number, date and place of birth, and name of at least one parent or legal guardian for each child.

A Travel Consent Letter is used when a child is travelling alone, with only one parent/legal guardian, or with an authorized group. This document notifies authorities or other interested parties that a parent or legal guardian has given their consent for the child to travel without them.

A notarized letter of consent will be required for children who travel without one or both of their parents or legal guardians. The letter of consent ensures the legitimacy of the travel consent offered by the accompanying adult.

A written and signed statement of the supervising adult certifying that he or she has parental or legal guardian consent for each child. CBP also suggest that this note be notarized, to easily verify the validity of the parental authorization. For frequent border crossers, the letter should not exceed one year.

While the Government of Canada doesn't require a consent letter for children traveling without both parents, they strongly recommend it.

Mention the name and address of the receiver properly. Include the subject line stating the permission for travel. Greet the receiver using Sir/Mam. The body of the letter should stick to the subject line and should sound polite.
