To receive CHDP services, children and youth must live in California and meet one of the following requirements: Up to 21 years of age and on Medi-Cal. Up to 19 years of age and from a low-income family. Between the ages of 3 and 5 years and attend a Head Start or state preschool.

What does a CHDP exam include? A well-child exam includes assessment of oral health, nutrition, development, tuberculosis testing, and education about ways to keep your child healthy. The exam also includes any immunizations that your child needs.

CHDP program offers preventive health services that identify and prevent disease and disabilities among low-income children and youth. CHDP will sunset on July 1st, 2024.

The CHDP program aims to prevent and identify health problems through regular, no cost health assessments and assure that children have access to ongoing health care. These health assessments are free of charge to eligible children.

The child must complete a health examination within 90 days of the date of entry into first grade in California. The School Entry Health Checkup Requirement form should be distributed at the beginning of the school year in registration packets.

Medi-Cal Children and Youth All Medi-Cal recipients from birth to age 21 are eligible for CHDP-scheduled periodic health assessments and services based on the federally-mandated Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) Program.

CHDP is a program that provides well-child check-ups to children enrolled in Medi-Cal as well as those children not qualified for Medi-Cal but determined income-eligible. CHDP pays for periodic exams so that problems may be found early. These problems can then be treated before they become more severe.
