A Florida SHOTS™ Form DH 680, certified with electronic signature, may be printed by enrolled health care providers and is also accessible to any school, licensed childcare facility, or daycare center enrolled in Florida SHOTS™.

Mandatory Immunization Requirements for the 2022-2023 School Year Students entering Kindergarten Fall 2022 must obtain the 2nd MMR (Mumps Measles and Rubella) and the 2nd Varicella vaccine. Students entering the 7th grade Fall 2022 must obtain the Tdap booster (tetanus diphtheria pertussis) vaccine.

Form DH 681, Religious Exemption From Immunization, is issued if immunizations are in conflict with the religious tenets and practices of the child's parent or guardian. This exemption is issued by a County Health Department (CHD) and based on established religious beliefs or practices only.

The Form DH 680, Florida Certification of Immunization, must be used to document receipt of Immunizations and is required for enrollment into the Florida school systems, programs, childcare facilities, and family daycare homes.
