Diagnostic criteria for nonviable pregnancy early in the first trimester. The revised criteria suggest that an embryo length of ≥7 mm with no heartbeat or an MSD of ≥25 mm with no embryo is diagnostic of pregnancy failure.

An hCG value below 500 mIU/mL can be considered as a sign of an abnormal pregnancy(9). Chung et al. ascertained that an intrauterine pregnancy existed in more than 50% of cases when hCG values were above the threshold and USG did not reveal an intrauterine sac.

Nonviable A pregnancy is nonviable if it cannot possibly result in a liveborn baby. Ectopic pregnancies and failed intrauterine pregnancies are nonviable.

These new criteria state miscarriage be considered only when: an empty gestation sac has an MSD of ≥ 25 mm (with no obvious yolk sac), or embryonic crown rump length ≥ 7 mm (the latter without evidence of fetal heart activity). If in doubt, repeating scans at an interval is emphasized.

Based on these early studies, a crown–rump length (CRL) of 5 mm without cardiac activity or an empty gestational sac measuring 16 mm in mean gestational sac diameter have been used as diagnostic criteria to confirm early pregnancy loss 10 11.

The term “early pregnancy loss” or EPF refers to a nonviable, intrauterine pregnancy with either a gestational sac that is empty or contains an embryo or fetus without heart activity within the first 12 6/7 weeks of gestation.

A blighted ovum, also called an anembryonic pregnancy, occurs when an early embryo never develops or stops developing, is resorbed and leaves an empty gestational sac. The reason this occurs is often unknown, but it may be due to chromosomal abnormalities in the fertilized egg.
