If you want to change a parent who was listed on the child's original birth certificate, that must be done through the court system. This is referred to as an “adjudication” – the court will decide (adjudicate) the existence or nonexistence of the parent and child relationship.

A voluntary declaration of parentage or paternity is a California governmental form that, when signed by both parents, establishes them as the legal parents of the child. The form must be signed voluntarily. No one can force either person to sign the form.

For unwed mothers only, if the father was not available or present at the time of birth, the parents must sign an Acknowledgement of Paternity form (VS22) in front of a qualified witness from the Department of Child Support.
