The Employee is required to report the injury to his/her supervisor as soon as possible after the incident/accident. The Supervisor/Lead is responsible: To assure that any injured employee is given immediate and proper medical care (as required) and that no one else can be injured.

There is a legal requirement to not only report what happened, but also the steps taken to prevent a recurrence. It is wise to investigate these events to see if they were the result of unforeseen, isolated circumstances, or if they resulted from a breakdown or oversight in the company's health and safety program.

All accidents/incidents involving UBC workers or practicum students must be immediately reported to the responsible supervisor and then on-line through UBC CAIRS.

Collect Evidence Collecting it for later examination is a crucial part of the OSHA incident investigation process. Collect witnesses. Identify any evidence and start sketching the scene that includes dimensions and placement of affected employees. Photograph the area and physical evidence from various angles.

Releasing the incident scene for further work. Analyzing collected data. Creating an accident report and sharing your findings. Determining corrective steps.

The first step in the investigation is to secure the incident scene and preserve any physical evidence using cones or other barriers to protect sensitive areas. If the investigation team has to travel a long distance to reach the scene, on-site management should be instructed to carry out this step.

Note: If a serious incident occurs at the workplace, a Safety and Health Officer may conduct an incident investigation. The Officer will require that items involved in the incident (e.g. equipment, materials) not be moved unless it is necessary to release an injured person or to avoid creating additional hazards.
