Follow the steps to fill out the Player Transfer Form online.

1. Click ‘Get Form’ button to obtain and access the Player Transfer Form, opening it in your preferred editor.
2. Begin by completing the Player Information section. Provide your name, address, phone number, guardian email, date of birth, city, zip code, and guardian signature. Ensure all details are accurate.
3. In Section 1, labeled 'Release', indicate your current team name, team ID number, and current coach's name. Obtain the current coach's signature and email; remember that this signature does not guarantee approval outside the transfer window.
4. If necessary, provide an explanation for your release in the designated space. If more room is needed, attach a letter.
5. Fill out the current certified registrar's name and ensure the team/club financial release is marked as required.
6. Move on to Section 2, labeled 'Transfer'. Check whether you are cleared or not cleared. When during the open transfer window, ensure the current coach's release is signed and submitted to the current registrar.
7. Enter the new team's name, team ID, and new coach's name. Secure the new coach's signature and email.
8. Complete the registrar’s name and circle the region designation.
9. Depending on whether you are filing during or outside of the transfer window, make sure to adhere to all applicable procedures as outlined by Utah Youth Soccer. If necessary, submit the completed form to the League Commissioner.
10. Once all sections are completed, save your changes, download a copy, print it, or share the form as required.
