A Caprini score of 10 or greater is considered high risk and a score of 9 or less is considered low risk. The 2013 version of the Caprini RAM retrospectively stratified 7 of the 8 VTE events correctly, while only 1 VTE was identified with the prospective department protocol.

Originally developed for surgical patients, the Caprini RAM facilitates the derivation of VTE risk by summing individual risk factors so as to place patients into four categories: “low risk” (0-1 points), “moderate risk” (2 points), “high risk” (3-4 points), and “highest risk” (≥5 points).

The Caprini risk score is a risk assessment tool for the occurrence of venous thromboembolism among surgical patients. The Caprini risk score includes 20 variables and it is derived from a prospective study of 538 general surgery patients.

The Padua prediction score, indicating a high risk, was defined as a score of 4 or higher. The Caprini VTE risk assessment identifies high risk as a score of 5 or higher.

A Caprini score of 10 or greater is considered high risk and a score of 9 or less is considered low risk.

Patients at low risk of VTE (Caprini 1 to 2) are recommended to receive either mechanical or pharmacological prophylaxis. Patients at moderate (Caprini 3 to 4) to high risk of VTE (Caprini 5) are recommended pharmacological prophylaxis either alone or combined with mechanical prophylaxis.

A Caprini score of ≥ 5 points was considered the criterion of a reliably increased risk of DVT in surgical patients with standard thromboprophylaxis. Predicting DVT using a cumulative risk score ≥ 4 is recommended for nonsurgical patients.
