The specificity increases in follow-up rounds to around 97%, with positive predictive values for biopsy in the same range as for mammography. MRI preferentially detects the more aggressive/invasive types of breast cancer, but has a higher sensitivity than mammography for any type of cancer.

An MRI may identify a small mass within a breast better than a mammogram or ultrasound. This is particularly true for very dense, non-fatty breast tissue. Learn more about a tumor that a doctor feels in the breast but cannot see on a mammogram or ultrasound.

Breast MRI has been demonstrated to have the highest sensitivity for breast cancer detection, ranging from 94% to 100%, and it can depict small invasive cancers measuring <5 mm. However, prior studies have shown that the ability of MRI to characterize a lesion as malignant increases with increasing size.

Breast MRI requires intravenous injection of gadolinium-based contrast to evaluate the breast tissue (Fig. 13), looking at both anatomy and blood flow patterns. Normal tissue, cancers, and benign (noncancerous) findings can enhance (light up) on MRI.

The nurse will administer the IV sedation and monitor your vital signs throughout the MRI scan. The sedation will help you relax, but it will not put you to sleep.

MRI, used with mammography and breast ultrasound, can be a useful diagnostic tool. Recent research has found that MRI can locate some small breast lesions sometimes missed by mammography. It can also help detect breast cancer in women with breast implants and in younger women who tend to have dense breast tissue.

Screening MRI is recommended for women with an approximately 20–25% or greater lifetime risk of breast cancer, including women with a strong family history of breast or ovarian cancer and women who were treated for Hodgkin disease.

Breast MRI remains the most sensitive modality for the detection of breast cancer, with a reported sensitivity ranging from 77% to 96%, higher than the sensitivity of either mammography or US (1–10). This holds true for invasive carcinoma as well as for ductal carcinoma in situ (11).
