The ocular motor systems control eye lid closure, the amount of light that enters the eye, the refractive properties of the eye, and eye movements. The visual system provides afferent input to ocular motor circuits that use visual stimuli to initiate and guide the motor responses.

Normal function of the vestibulo-ocular reflex (VOR) coordinates eye movement with head movement, in order to provide clear vision during motion and maintain balance.

What Is the VOMS Test? VOMS is a tool designed by the experts at UPMC to detect signs and symptoms of a concussion. It stands for vestibular ocular motor screening and looks at the systems in charge of integrating balance, vision, and movement.

Neuro-ocular vestibular dysfunction is an illness involving the neurologic, ocular and vestibular systems. It includes the signs of motion sickness such as nausea, headache, and/or dizziness, and photophobia or visual sensitivity to motion and headache. It is usually overlooked in the standard eye examination.

Ocular motor impairments and symptoms may manifest as blurred vision, diplopia, impaired eye movements, difficulty in reading, dizziness, headaches, ocular pain, and poor visual-based concentration.

Sit facing the patient. Hold 1 finger 3 feet below the other, and about 3 feet from the patient (so that the patient gazes 30° up and 30° down). Ask the patient to move their eyes from point to point as quickly as possible. Perform 10 times.

The VOMS consists of five measures of ocular and vestibular function to include; smooth pursuits, saccades, near point of convergence (NPC), vestibular ocular reflex (VOR) and visual motion sensitivity (VMS).

The vestibulo-ocular reflex (VOR) is a gaze stabilizing reflex: the sensory signals encoding head movements are transformed into motor commands that generate compensatory eye movements in the opposite direction of the head movement, thus ensuring stable vision.
