You must submit these documents to your Employer or SSS at least 60 days from the date of conception. For employed members, once you submitted these documents to your employer (or through your HR officer), your employer will then immediately notify SSS about your pregnancy.

What if I lose my MATB1 form? You can ask your midwife or doctor for a duplicate form but there may then be some delay before you get it. To avoid this happening and potentially risking being late to apply for SMP, make at least 1 copy of your MATB1 form when you first get it, and keep that copy somewhere safe.

How and when do I get my MAT B1 form? Your midwife or doctor should give you your MATB1 form when you reach your 20 week mark or soon after when you have your routine appointment. They cannot issue the form any earlier than this.

You'll be fine, it's very common for women not to get hold of the form until after 25 weeks due to how their midwife appointments fall, whether the midwife has run out of the forms at the time (happened with DS1) and so on.

There are two ways to submit your maternity notification: over-the-counter at an SSS office, or online through the SSS website.

To apply for it, you'll need to be 26 weeks pregnant and you'll need to fill in a MA1 claim form – you can either fill it in online or print it out then fill it in. Either way, you'll need to print it out and send it to the address on the form.
