Our Mission The mission of Johns Hopkins Medicine is to improve the health of the community and the world by setting the standard of excellence in medical education, research and clinical care.

The ACG risk adjustment system Each ADG is a grouping of diagnosis codes similar in terms of severity and likelihood of persistence of the health condition treated over a relevant period of time, typically 1 year. ADGs are not mutually exclusive and individuals can have multiple ADGs (up to 32).

Those values – respect, dignity, integrity, inclusion, excellence, and diversity – still ring true today.

The ACG System provides a measure of patient sharing, termed “Care Density Score” based on this research. This patient-level measure indicates how much of the care of that patient is shared among clinicians.
