Electronic and mailed transcripts must be order through National Student Clearinghouse at https://tsorder.studentclearinghouse.org/school/ficecode/00201700 for student who attended after 1985. For each request, there is a $2.50 processing fee (paid by the student at the point of service).

To change your curriculum, you must obtain the form from your current or new advisor or department. The form must be signed by your current department and the department to which you want to move; both academic department heads involved must approve the change.
