Any child entering 6th grade is required to show proof (see section 665.250(b) of receiving one dose of Tdap vaccine regardless of the interval since the last DTap, DT or Td dose, and one dose of Meningococcal vaccine. Any child entering 6th grade is required to show proof of having 3 doses of Hepatitis B.

Children who are not vaccinated can transmit vaccine- preventable diseases at schools and in the community. Unvaccinated children can infect babies who are too young to be fully immunized. Unvaccinated children can infect people of any age who can't be immunized for medical reasons.

New students enrolling in an Illinois school for the first time will be required to obtain a physical examination, vision exam, and provide up-to-date immunization records regardless of grade. If your child needs to take medication during school, a new medication authorization form is needed each school year.

The State of Illinois requires parents/guardians must provide proof of required immunizations and school physical exams before October 15, 2023, or their child will face exclusion from school. For more information about CPS health requirements, contact your School Nurse.

Vaccination Requirements The State of Illinois requires vaccinations to protect children from a variety of diseases before they can enter school.
