It is a shipper's responsibility to provide the right documentation to their carriers. In addition to the shipping papers, carriers must also have the emergency response information which can come handy in the case of an incident.

When must the DD Form 626 be signed? Inspection form DD 626 must be completed for every vehicle that leaves or arrives at military facilities. The inspection consists of the mechanical aspects of the motor vehicle and all cargo spaces where the hazardous material will be stored for transport.

DD Form 626, Motor Vehicle Inspection (Transporting Hazardous Materials), Figure J-6, will be used for inspecting both commercial and government vehicles carrying placarded amounts of HAZMAT on public highways.

(1) The material must be in proper condition for transportation ing to the requirements of this subchapter; (2) All packages in the transport vehicle or freight container must be secured to prevent shifting in any direction.

DA FORM 2404, FEB 2011 OR PENCIL - Indicates that a completely satisfactory condition exists.
