Get Aesthetic Consent Form

A document with important information about a medical procedure or treatment, a clinical trial, or genetic testing. It also includes information on possible risks and benefits. If a person chooses to take part in the treatment, procedure, trial, or testing, he or she signs the form to give official consent.

Informed consent is the process in which a health care provider educates a patient about the risks, benefits, and alternatives of a given procedure or intervention. The patient must be competent to make a voluntary decision about whether to undergo the procedure or intervention.

Defining consent informed – the person must be given all of the information about what the treatment involves, including the benefits and risks, whether there are reasonable alternative treatments, and what will happen if treatment does not go ahead.

Informed consent remains valid for an indefinite period, allowing advance consent to be sought, providing that the patient's condition has not changed, and/or new information concerning the proposed intervention or alternative treatments have not come to light in the intervening period.

How long is a consent form good for? Generally, there is no strict rule on how long in advance of the procedure informed consent can or should be obtained. Although some procedures have specific requirements (e.g. sterilization). Otherwise, physician discretion and hospital policy govern.

The UK GDPR does not set a specific time limit for consent.
