Certain people are at higher risk of developing more serious complications from COVID-19, including older adults and those with underlying medical conditions such as heart or lung disease, chronic kidney disease requiring dialysis, liver disease, diabetes, immune deficiencies, or obesity.

Possible symptoms include: Fever or chills. Cough. Shortness of breath or difficulty breathing. Fatigue. Muscle or body aches. Headache. New loss of taste or smell. Sore throat.

Use one sheet per day. Each worker must be asked about all of the following symptoms: fever, cough, shortness of breath, sore throat, loss of sense of smell and change in sense of taste. Also ask if s/he has been been told that s/he has COVID-19 in the past 2 weeks or if s/he is waiting for a COVID-19 test result.
