Get Temporary Placard 2013-2024

Complete the Application for Temporary Placard (Form SP-68) or visit a local, municipal police department to obtain the application. Obtain certification from a qualified medical practitioner that you are qualified for a temporary placard; see Qualifications for medical certification information.

The temporary placard is valid for no longer than 6 months from the date of issue and can only be re-certified once, for a period not to exceed 6 months. The fee for a placard is $4.00. Payment must be a check or money order payable to "NJMVC".

How to apply. To apply for plates or a placard, complete the Application for Vehicle License Plates and/or Placard for Persons with a Disability (Form SP-41). There is no fee for either the license plates or the placard. A copy of the vehicle registration must be included with the plate application.

Who's eligible? has lost the use of one or more limbs as a consequence of paralysis, amputation, or other permanent disability; is severely and permanently disabled and cannot walk without the use of or assistance from a brace, cane, crutch, another person, prosthetic device, wheelchair or other assistive device;

Per NJ Law, when parking at a metered space, a driver with a placard or handicapped license plate must put the appropriate amount of money in the meter.

A “Person with a Disability Identification Card” is issued with the license plates and placard. This ID card must be carried by the person with a disability in conjunction with the use of the license plates/placard. RECERTIFICATION APPLICATION: Medical recertification is required every three years.

Generally, the MVC requires four to six weeks to issue plates and placards; additional time should be allowed for mailing.
