This Adult ADHD Self-Report Screening Scale for DSM-5 (ASRS-5) is intended for people aged 18 years or older.

Score each item in the range 0-4 and give everyone a summary score of 0-24. A score of 14 or higher is suggestive of ADHD (91.4% sensitivity, 96.0% specificity).

A widely used screening instrument for identifying adult ADHD is the Adult ADHD Self-Report Scale, version 1.1 (ASRS-v1. 1). Using a total score of ≥4 on the six-item Part A section of the scale as the threshold to indicate a positive screening test, the developers of the ASRS-v1.

Scoring and Interpretation If the respondent scores 4 or more in Part-A, then the symptom profile of the individual is considered to be highly consistent with an ADHD diagnosis in adults (Adler et al., 2006; Kessler et al., 2007).

The Adult Attention-Deficit/Hyperactivity Disorder Self-Reporting Rating Scale (ASRS) is composed of 18 questions, and uses a scale that ranges from 0-4 based on the individual's mark in either the "never, rarely, sometimes, often, very often" column for a possible total score of 72.
