I, the parent/guardian of the student named above, hereby give permission for my child to participate in team activities, as directed by the school/coach. I understand that there is an inherent risk of injury in sports, particularly with contact sports.

I fully understand the risks associated with exercise, including the risk of bodily injury, heart attack, stroke or even death, but knowing these risks, it is my desire to participate as herein indicated. I understand that this program may or may not benefit my physical fitness or general health.
