Get 1095 B

Do not attach Form 1095-B to your tax return - keep it with your tax records. Certain employees of applicable large employers (See next column). Form 1095-C provides information about the health coverage offered by your employer and, in some cases, about whether you enrolled in this coverage.

Health care coverage documents You are not required to send the IRS information forms or other proof of health care coverage when filing your tax return. However, it's a good idea to keep these records on hand. This documentation includes: Form 1095 information forms.

The person or their tax preparer will use the information from Form 1095-B as proof of health coverage for that tax year. ​If you only had health coverage for part of the year or no health coverage at all, the tax penalty no longer applies.

If you miss or forget to file on time, you will definitely be fined by the IRS. It applies to all forms, including Form 1095-B.

Form 1095-B, Health Coverage, is provided by insurance companies and other coverage providers. However, if your coverage was insurance purchased through the Marketplace or was a type of coverage referred to as “self-insured coverage” that was provided by an applicable large employer, you will receive a different form.

If you got a “Corrected” Form 1095-B after you filed your taxes, you may need to amend your taxes for the year noted on your Form 1095-B. DHCS will send the information on your Corrected Form 1095-B to the IRS and the Franchise Tax Board (FTB). Keep this Form 1095-B for your records.
