Form 8850 is an important pre-screening step that employers need to fill out and submit to their SESA if they want to ensure that the employee they are looking to hire, is a certified member of a targeted group or a long-term recipient of family assistance.

Employers use Form 8850 to pre-screen and to make a written request to the state workforce agency (SWA) of the state in which their business is located (where the employee works) to certify an individual as a member of a targeted group for purposes of qualifying for the work opportunity credit.

For purposes of this paragraph, the term “pre-screening notice” means a document (in such form as the Secretary shall prescribe) which contains information provided by the individual on the basis of which the employer believes that the individual is a member of a targeted group.
