Empty follicle syndrome (EFS) is a condition in which no oocytes are retrieved from the mature follicle after ovulation induction in in vitro fertilization (IVF) cycles. This syndrome was first reported by Coulem et al,1,2 in 1986.

Sometimes, we may also get more eggs than the number of follicles counted on the scan. This is because doctors may not bother to diligently count all the follicles seen on the ultrasound scan; or that some follicles are hiding behind the others, which is why they may not be seen during the scan.

The follicle may or may not contain an oocyte and the oocyte can be abnormal or normal, mature or immature. It is NOT UNTIL AFTER FOLLICULAR ASPIRATION THAT WE CAN ACTUALLY SEE THE OOCYTE ITSELF. The oocyte is a cell and, therefore, it is microscopic. This is why it needs to be viewed in a laboratory.

WHAT MAYBE THE REASONS FOR EMPTY FOLLICLE? The follicle may have ruptured (ovulated) prior to the egg collection. This may happen if the trigger injection is mistimed (i.e. given earlier than instructed) ; or if the egg collection is delayed beyond 37 hours of trigger injection administration.

Seeing more follicles on ultrasound means there are even more follicles that can't be seen. As a result, a higher count means a greater ovarian reserve. A higher number also indicates that you would likely respond well to ovarian stimulation medications.

When Should a Follicle Scan Be Done? A follicle scan should be done several times during a woman's menstrual cycle, usually from days 9-20. A basically painless procedure, a follicle scan lets the doctor see the development of the follicle within the ovary.
