Get apply for disability parking permit 2018-2024

You and your healthcare provider can complete the Application for a Parking Permit or License Plates, for Persons with Severe Disabilities (PDF) (MV-664.1). You also can have your healthcare provider (MD, DO, DPM, or NP) complete a statement on their letterhead.

You must be a Nassau County Resident. You are eligible for a permit if you are a resident of Nassau County who has one or more severe disabilities that impair your mobility. All applications must have a Nassau County street address (no post office boxes). All applications must be fully completed and properly executed.

You may apply for a placard and/or plate at your local county tax assessor-collector's office.

State law requires every city, town and village to appoint an agent (usually the city, town or village clerk) to issue parking permits for people with disabilities. Contact your city, town or village clerk to find out which local agency issues the permits. There is no fee for the permit.

The State Parking Permit for Persons with Severe Disabilities (known as the "blue hang tag") is now valid for 5 years from the date it was issued. After 5 years, the permit expires and you must re-apply by submitting a new application to the Department of Transportation (DOT) by mail.
