This form must be signed by the last day of the calendar month following the month of supervision, and both you and your supervisor must retain copies. Once you've completed all of your fieldwork and met all of the requirements, your supervisor will sign your Final Fieldwork Verification Form.

You can do a combination of supervised and concentrated. Hours accrued under the concentrated supervised fieldwork are accrued at a value of 1.33x the supervised fieldwork. Therefore, use a multiplier of 1.33 for any months that you accrue hours per the concentrated fieldwork requirements.
