School District Employer's Annual Reconciliation of Tax Withheld, Form SD-141 All employers required to withhold must file no later than January 31 of the next calendar year an annual report, Form SD-141, reconciling school district tax withheld and paid for the past calendar year.

Employee's Withholding Exemption Certificate IT 4. Page 1. Submit form IT 4 to your employer on or before the start date of employment so your employer will withhold and remit Ohio income tax from your compensation. If applicable, your employer will also withhold school district income tax.

Form IT-1140 is a withholding return and needs to be completed for all qualified investors. The instructions give a detailed list of who isn't a qualified investor, one of which is any partner included in the composite return (IT-4708). Resident partners won't get withholding.

Categories of workers that are exempt from Ohio withholding taxes include agricultural workers, domestic service workers, providers of low-income services, foreign government services, underage paper deliverers, and non-trade services.

Any individual (including retirees, students, minors, etc.) or estate that receives income while a resident of a taxing school district is subject to school district income tax. Individuals who work, but do not live, in a taxing school district are not subject to the district's income tax.

Every full-year resident, part year resident and full year nonresident must file an Ohio tax return if they have income from Ohio sources. An exception is for full year nonresidents living in a border state will not have to file an Ohio tax return if wages received are from an unrelated employer.
