You may apply for a placard and/or plate at your local county tax assessor-collector's office.

In order to obtain a placard, Form MV-145A (PDF), "Persons with Disability Parking Placard Application" must be completed by the person with a disability, a health care provider, or a police officer and returned to PennDOT. Permanent placards are valid for five years and renewed in March or September.

You can either apply for a renewal for your permanent placard by mail to the Department of Finance and Administration, by calling the toll-free number 1-800-941-2580, or online. You will need your renewal reminder, which has your renewal ID number and verification code.

Qualifying Conditions For Disabled Parking In Arkansas Cannot walk more than 100 feet without needing to stop to rest. Cannot walk without the use of or assistance from a brace, cane, crutch, another person, prosthetic device, wheelchair, zimmer frame, or other assistive device.

To obtain a handicapped parking placard or plate, you must present form 10-336, completed and signed by your own physi- cian.

The applicant's physician must complete the form VTR-214. The physician's signature must be notarized unless a separate written original prescription is submitted. There is no fee for permanent placards and a $5.00 fee for temporary placards.
