(1) File a Petition for Appeal (Form 2), within thirty days of entry of the family court final order. (2) The Petition for Appeal must be typewritten or legibly handwritten. You must file the original and two copies in the circuit clerk's office where the final order was entered.

An appeal from a final order of a family court may not be filed in the Supreme Court unless, within fourteen days after entry of a family court final order, both of the parties file a notice of intent to appeal directly to the Supreme Court and waive their right to appeal to the Intermeidate Court.

In accordance with Appellate Division Rule 670.3, one original and two copies of the following documents are to be filed with the clerk of the Family Court: Request for Appellate Division Intervention Form (RADI); Notice of Appeal; A copy of the order you wish to appeal (F-99 or Clerk's Certificates are not acceptable).
