An Affidavit of Support, also called the Form I-864, is a document an individual signs to accept financial responsibility for the applicant who is coming to live in the United States.

The beneficiary should include it with their non-immigrant visa application or take it to the U.S. consulate where they will apply for a visa or humanitarian parole. The entire Declaration of Financial Support package (Form I-134 and all supporting documents) must be submitted in one package.

Other individuals who may need Form I-134 are those coming to the United States as a tourist or on a visitor visa. The I-134 affidavit of support is an eight-page document that is filled out by your U.S. sponsor.

The completed forms and supporting documents must be submitted in CEAC to NVC with the visa applicant's civil documents.

We recommend completing Affidavit of Support forms on a computer or typewriter, with answers typed in CAPITAL letters. After the form is completed, upload, and submit the signed form to NVC via CEAC along with the sponsor's supporting financial evidence.

The affidavit of support is a legal contract between the sponsor and the U.S. government that states the sponsor is financially responsible for the person coming to the U.S. on a visitor visa (which includes the K1 and K2 visa).

Proof of Sponsor's Funds Bank statement or other financial documents including deposits, date the account opened, total amount deposited for the past year, and present balance. List containing serial numbers and denominations of bonds and names of record owner(s).
