Complete the affidavit: Fill out the form with the decedent's information, a list of their assets and the names and addresses of all heirs or beneficiaries. Notarize the affidavit: Sign the completed affidavit in front of a notary public. Some states might also require additional witnesses.

A father's name cannot be added or deleted from a Birth Certificate through the Probate Court. To add a father or delete a father from a Birth Certificate when the parents are not married, you must contact the Child Support Enforcement Agency.

Ohio law permits an adult to apply to the Probate Court for a name change. The filing must be made in person. After the filing of the paperwork the Court will set a hearing date, generally 60 days after the filing.

Except for motions made during a hearing or during trial, motions are required to be in writing. They must also be filed in court and served on the opposing party's attorney (or the party if not represented by counsel).

Probate is required regardless of the value of the estate. There are also several types of so-called non-probate property, which pass outside of Ohio probate.
