Standard Form 95 is used to present claims against the United States under the Federal Tort Claims Act (FTCA) for property damage, personal injury, or death. The Standard Form 95 requires you to complete the amount of the claim for which you are submitting as well as additional information.Claimants who wish to file an administrative tort claim against CBP should complete a Standard Form 95, Claim for Damage, Injury, or Death and submit it. The claim must be for a sum certain and should be filed out on the standard preprinted Form 95 entitled "Claim For Damage, Injury, Or Death. You must first complete an FTCA form or Standard Form 95 to submit your claim. A Standard Form 95 is frequently used to present claims against the U.S. under the FTCA. If you believe you have a claim against the FDOT, you may submit your claim electronically through the Claims Management System. Using a SF 95 form will help ensure that you've included all of the necessary information. The agency has six months to respond.
