``Jurors who fail to respond or appear for jury duty, without a reasonable excuse, may be punished for contempt of court pursuant to NJSA 2B: 20-14b and subject to a fine of up to $500.00 or imprisonment.''

Participating in our judicial system is not only an honor, but is also one of our civic duties. Although juror service may be inconvenient, citizens must participate in the jury process in order for it to work.
If you have a compelling reason to be excused from jury service (extreme inconvenience or undue hardship), you must notify the jury office in writing. Any request must be prepared by the juror and detail the hardship. You may include any attachments you deem appropriate to aid the Court in rendering a decision.
When writing a jury excuse letter, directly state why you cannot serve and how serving would lead to hardship. Valid exemptions include being ill or disabled, being the sole guardian of young children, or being over the age of 70. Include documentation such as a doctor's note or a letter from an employer.
They usually do not require you to attend if you tell them you are seeing a therapist or doctor regarding your concerns. I see that you are being counseled currently and that you are also prescribed medication. If you inform them of this, they should be able to excuse you with little or no further questions asked.
