Letter of recommendation formats depends on the company and job description. Regardless of your relationship with the applicant, it's important to include a name and date at the top of the page.
Never use st, nd, th, or rd with a date. A date is always written with month (abbreviated if appropriate) and day without a superscript. If the date includes a year, use a comma after the number before the year.
This format puts the year first, followed by the month and then the day, i.e. YYYY-MM-DD. For example: 2023-04-12 would be 12 April 2023. 2024-08-05 would be 05 August 2024.
