Thanks to Direct Access legislation, physical therapy patients can now get evaluations and treatments directly from a licensed physical therapist (PT) without first obtaining a doctor's prescription or referral. It essentially means anyone can visit a physical therapist whenever they want.
Leave a signature. Include both addresses. At the top of the letter of referral, include your address, contact information, the date and address of the recipient. Write a brief introduction. Give an overview of the applicant's strengths. Share a story of the applicant. Add a closing statement. Leave a signature.
Talk about how they listened and cared and how they helped you. Be specific - that will add to the authenticity and value. Even better if you have had PT elsewhere mention how this was better (but don't mention the other clinic name).
