Download the Personal CORI Request Form from the Department of Criminal Justice Information Services (DCJIS) website. Or call DCJIS at 617-660-4600 (for TTY, call 617-660-4606) to get the form by mail. It is important to print neatly. DCJIS may send back forms that are hard to read.

If you have a valid Massachusetts I.D. or driver's license and are not submitting an indigency waiver, you may submit your CORI request online at Mass/CJIS. This form is only to be used to request your own personal CORI information.

Generally: For a basic background check through the California Department of Justice, the average processing time is 1-2 weeks. For fingerprint-based background checks required for certain professions or licenses, the processing time is typically 4-6 weeks.

Employers usually access CORI separately from a background check report. If the law requires your company to search CORI, you must do so in addition to your Checkr search.
