Form DS-11 must be typed or completed in black ink. Do not sign the form. Form DS-11 can be downloaded from the U.S. Department of State web site. Passport Photo One 2" x 2" color photo taken within the last six months.

When done manually, it should be completed using a black ink pen.

Acceptance facilities include post offices, clerks of court, public libraries, and other local government offices which accept passport applications on behalf of the U.S. Department of State.

Proof of identity A valid driver's license or state-issued identification card. A previously issued U.S. passport book or card. Government or military identification. A Certificate of Naturalization or Citizenship. Trusted Traveler IDs (Global Entry, FAST, SENTRI, or NEXUS)

If you plan to travel internationally by air or to countries that require a visa, a passport book is more important. If your travel is limited to specific regions by land or sea, a passport card may suffice. Many travelers opt for both for maximum flexibility.
