Out-of-State Immunizations must be transferred to a Florida Certificate of Immunization Form (DH680). This form must be completed by a physician or a Florida County Health Department and electronically certified.

Immunization Requirements for Entry into Florida Schools Students from Pre-kindergarten through 12th grade who are coming into a Florida school for the first time must present an immunization record (DH 680) and a current Florida School Entry Physical Examination, Form (DH 3040).

Medical. A formal Medical Exemption may be completed for persons with certain medical conditions and/or contraindications (permanent or temporary) to certain vaccines. A letter from the student's physician or the official State of Florida Medical Exemption form must be submitted to the SHCC.

The Form DH 680, Florida Certification of Immunization, must be used to document receipt of Immunizations and is required for enrollment into the Florida school systems, programs, childcare facilities, and family daycare homes.

Immunization Services RELIGIOUS EXEMPTIONS (DH Form 681) - Individuals requesting Religious Exemptions for children need to apply in person and bring the child's birth certificate and parent/guardian's ID. Religious Exemptions do not require an appointment.
