Persons seven years of age or older who seek to correct their birth certificate must provide two documents from the long list found on the form. To obtain a birth certificate, complete and mail the application with your payment, a self-addressed envelope, and a photo copy of your Identification. The public may apply for a death certificate online at the Maryland Department of Health Vital Records page. If you were born in another state, you can call the health department at 301-600-1029 and we can give you the telephone numbers for the other states. You must go to the Vital Records location in Baltimore City for any corrections or changes to a birth certificate. Certified copies of birth certificates are available at the Health Department for births recorded in the State of Maryland from 1939 to present.
