Lender Agreement (Federally Insured Depository Institutions, Federally Insured Credit Unions, Farm Credit System Institutions) The SBA posted a sample application for seeking a PPP loan, along with instructions for completing and submitting the application (link expired). All participating Lenders must have a signed SBA Form 750, SBA Form 3506 or SBA Form 3507 (depending on type of Lender). This portal will allow you to complete the forgiveness application and submit all necessary documents to the SBA.
