If you are driving in NY and are involved in a crash you must file an MV-104 (Motorist report) if any of the following occurred. You must fill in all information requested on the report.You can also request a collision report online using the Collision Report Retrieval Portal. This online resource allows motorists to access collision reports. In New York, the form required for reporting a motor vehicle accident is the MV104 form. However, if they do not, New York law requires the drivers involved to file form MV-104 with the DMV no later than 10 days after the accident. You must complete Form MV104 to the best of your ability, and you must be careful to only provide accurate information. Submit the completed accident report form to the New York State DMV. Not all collisions must be reported at the time they occur, but even so, filing an accident report is to your benefit. The MV104 must be filed within 10 days of the date of the accident.
