The notice shall include the name and post office address of the judgment creditor and the judgment creditor's lawyer, if any, in this State. Notice to Debtor: 1. Fill out this form and bring it with you to court; AND. 2. The Judgment Creditor must write in the name and address of the person directed to appear.
