Complete the contract request packet.Hennepin County Sentencing to Service (STS) is a structured work program that provides a sentencing alternative for low-risk adult and juvenile offenders. Find information and supports for contracted employment service providers who work with eligible county residents to build pathways to sustainable careers. Register as a non-contracted provider. Non-contracted providers do not have signed agreements with us; they are considered out of network. Complete the required documents and fax them to MHCP's Provider Eligibility and Compliance division. Only enrolled Housing Stabilization Services providers may submit this form to DHS. All submitted forms are reviewed as soon as possible, in the order received. Personal care assistance (PCA) services provide assistance and support for people with disabilities who are living independently in the community.
