The Federal Rules of Civil Procedure state that no certificate of service is required when a paper is served by filing it with the court's electronic-filing (ECF) system.

If using a custom form, be sure to cover essential information, such as: Case name and number. Description of documents served. The server's full name, contact details, and signature. Name of the person served. Time, date, and location of service. Confirmation that the server is over 18 and not a party to the case.
The proof of service must be signed by the process server under penalty of perjury, affirming the accuracy and truthfulness of the statements therein. And the affidavit is signed by the process server in front of a notary who puts them under oath that their statements therein are accurate and truthful.
The purpose of a proof of service is to list the parties and attorneys who were served and list the method of service -- by Notice of Electronic Filing, by mail, by overnight mail, or by email.

You cannot send more than 30 questions to another party in a circuit court case. The Maryland Rules have form interrogatories that you can use as an example.The court requires proof of service. The person who served the documents should fill out an affidavit of service. Fill out, sign, and submit a certificate of service to the clerk, including each of the parts below. "Discovery" is a process you can use before your trial to find out information from another party. There are several ways to ask the other party questions. Parties exchanging discovery material are encouraged to comply with requests that the material be provided in a word processing file or other electronic format.
