ISPA is the Ontario law that requires students attending public or private schools to have up-to-date vaccinations or a valid statement of exemption for medical reasons or reasons of conscience or religious belief.
View your immunization record online To log in to view your records online at Immunization Connect Ontario (ICON), you will need your health card number. If you need support, or need an Ontario Immunization Identification Number (OIID), please call KFL&A Public Health at 613-549-1232.
ISPA uses a professional audit assessment for individual evaluation. Upon ISPA receiving the application, it will be passed on to the ISPA Selection Committee for review.
The Immunization of School Pupils Act requires that all children under the age of 18 registered in school in Ontario must be immunized, or in the process of becoming immunized. Under the Act, parents are directly responsible for the immunization status of their children.
Under the "Immunization of School Pupils Act (ISPA)", children attending school in Ontario are required to be vaccinated against: Diphtheria. Tetanus. Pertussis (whooping cough) Polio. Measles. Mumps. Rubella. Meningococcal.
The medical exemption requires a signature from a licensed physician in the state of Oklahoma. The religious exemption requires a signature from a parent or a religious leader. A personal belief exemption requires the signature of a parent and a brief summary of the objection.
Children whose parents object to immunizations based on religious teachings or personal beliefs may seek an exemption. Lost immunization records are not grounds for an exemption. Parents who have lost their child's records should contact their local health department or family physician/health care provider.
All children two months of age and older must present an immunization record or file for an exemption before they are allowed to attend child care or school in Oklahoma.
