Most children must appear in person at a passport acceptance facility to apply for a passport. Please NOTE: The District Clerk no longer processes U.S. Passport Applications. Step 1: Complete and print the DS-11 form. Do not sign the form. Application form DS-11: This form must be filled out but not signed until you submit it at the post office. If the applicant is under 16 years old, both parents and the applicant MUST be present at the time of application. Complete and submit Form DS-11 to a Passport Acceptance Facility.
