Application form DS-11: This form must be filled out but not signed until you submit it at the post office. Passport and Westchester County ID applications are now accepted five days a week. For your convenience, documents may be delivered to the County Clerk. You can submit your US passport application at these King County District Court locations. An appointment is required.
