Most applicants who are age 1617 must apply in person using Form DS11. Passports for applicants 16 and older are valid for 10 years.DS11: Application for U.S. Passport. Like adult applicants, 16 or 17-year-olds need to provide proof of U.S. citizenship, a valid photo ID, and a passport photo. Provide one passport photo; Provide Social Security Number (not the actual card); Two forms of payment are required. Birth certificates must be submitted even when renewing a passport for a seventeen (17) years of age or younger. All visitors must have the application (DS-11)filled out (black ink only, no corrections and not signed), along with necessary documents, before checking in.
